Policy for Missed Appointments

The fee schedule for missed appointments is as follows:

  • Regular Office Visit — $50.00
  • Scheduled Procedure — $100.00
  • Hospital Procedure — $150.00

Appointment Information

For your convenience, Northern Virginia Urology accepts most area insurance plans. Please bring your insurance card with you to your appointment, and if your insurance requires a referral from your primary care physician bring this too. If you do not know whether or not you need a referral, the back of your card will usually say so. Otherwise, call your insurance company prior to your visit.

Please arrive early for your first appointment to have time to complete the necessary patient history and insurance forms.
